Food questionnaires
A food questionnaire records how often a client eats particular foods, rather than what they ate on specific days. Use it for habitual intake, where a diet record would only capture a snapshot.
Food questionnaires live alongside diet records in the same list on the Diets step of the client wizard. They are available for clients only; client groups have diet records only.
Add a questionnaire
- On the Diets step, click New, then Food Questionnaire. A questionnaire is added to the tree.
- Click its name to rename it inline.
- Expand the questionnaire to see its three frequency buckets: Daily, Weekly, and Monthly.
Add foods
- Select one of the frequency buckets using its chevron. The Questionnaire button only becomes available when a bucket is selected, not when the questionnaire itself is selected.
- Click Questionnaire. The Add Questionnaire dialog opens.
- The dialog lists the standard food categories:
- Breads, Cereals and Grain Products
- Fruits and Juices
- Fats and Oils
- Milk, Yogurt and Cheeses
- Vegetables
- Beverages
- Protein Foods
- Desserts and Sweets
- Miscellaneous Foods
- Expand a category to see its foods. Use the Food Name box to search across them.
- Select the foods the client eats, enter a Frequency for each, and choose an Interval of Daily, Weekly, or Monthly.
- Click Save. The foods appear under the matching bucket in the tree.
The frequency and the interval work together: a frequency of 3 with an interval of Weekly means the client eats that food three times a week.
Change a food's frequency
Select a food under one of the buckets and click Properties. The dialog shows the Frequency along with read-only nutrient and exchange values for that food.
Analyze a questionnaire
Select the questionnaire in the tree and click Analysis. The dialog compares the reported intake against a nutrient goal template, the same way it does for a diet record. The day range option does not apply here, because a questionnaire has no days.
Print a blank questionnaire
If you would rather have the client fill the questionnaire in on paper, print the Food Questionnaire Form report. It is a blank questionnaire with the client's information at the top and all of the standard foods listed by category. Enter the client's answers afterwards using the steps above. See client reports.
Remember to click Continue to save the questionnaire before leaving the step.
